Fill out Installation Reports (Form Ts) online


A fillable Installation Report/Form T is now available online for Installers to fill out, print (to send in with payment), save, and email to the appropriate MHD field office, all in one effort.

Using this feature is a convenience for license holders and also enables the MHD to inspect the installation as quickly as possible.

To use the form:

  1. Open Form T - Notice of Installation (PDF).
  2. Complete the form.
  3. Select the "Check Status and Print" button at the top right.
  4. Choose your email type*.
  5. Choose the appropriate field office.
  6. Select the email button.
  7. Save the completed form.

A confirmation of receipt titled "Form T Received" will be emailed back to you. If you do not receive a confirmation within 15 minutes, contact us at 877-313-3023.

* Note: If you are using Web-based email (like Yahoo! or Gmail) rather than installed email software (like MS Outlook), follow the instructional note next to the field office selection box on the form.
