Form Revisions Adopted

The following form revisions were recently adopted by the Manufactured Housing Division Board and are effective July 3, 2011.  All outdated forms should be replaced and will no longer be accepted after December 2011.

• Application for Statement of Ownership and Location
• Release or Foreclosure of Lien (Form B)
• Application for License Renewal (other than a salesperson)
• Notice of Installation (Form T) for Provisional Installer's License
• Application for Salesperson's License Renewal
• Field Verification Inspection Request Form
• Adding or Deleting a Related Person to a License Record
