The 2013 Emergency Solutions Grants Program (ESG) awards were approved by the TDHCA Board of Directors on July 25, 2013. 2013 ESG Subrecipient Submission Documents
All 2013 ESG Subrecipients must complete the following documentation prior to contract execution. Before completing and submitting these forms, please review the Instructions for Submitting Final Contract Documents (PDF).

|Application for Texas Identification Number Form (PDF on www.window.state.tx.us)||Only 2013 ESG Subrecipients that have never had a contract with the Community Affairs division of TDHCA must submit this form.|
|Direct Deposit Authorization Advance Payment Notification Form (PDF on www.window.state.tx.us)||Only 2013 ESG Subrecipients that have never had a contract with the Community Affairs division of TDHCA must submit this form, while those that have had previous contracts do not need to submit this form unless their bank information has changed.|

All required 2013 ESG Subrecipient documents must be submitted by 5:00 p.m. on Friday, September 6, 2013 for the contracts beginning October 1, 2013. All requested documents must be submitted by scanning them into one single PDF file and uploading them using the ESG Final Contract Document Submission form.
